Wayne Frazier

Mr. Frazier retired from NASA HQ where he served as Senior Safety and Mission Assurance Manager in the Office of Safety and Mission Assurance (OSMA). In this position, he was responsible for coordinating and integrating activities between OSMA and the NASA Engineering and Safety Center (NESC), NASA’s Software Independent Verification and Validation (IV&V) facility in Fairmont, West Virginia, Office of the Chief Engineer, and OSMA funded independent assessment functions at various NASA Centers. Mr. Frazier was a charter member of the HQ OSMA office formed after 51-L (Challenger) where at various times he has managed operational safety, institutional safety, and system safety groups.

Prior to joining NASA, Mr. Frazier worked with the Department of the Army culminating with 3 years on the Army Staff in the Pentagon where he worked on system safety, nuclear weapons design safety, chemical warfare safety, aviation safety, range safety, and propellant/explosives safety. Currently, Mr. Frazier is employed as a Senior Advisor for Alphaport, Inc, a knowledge management and engineering consulting company supporting the aerospace community out of Cleveland, OH. Mr. Frazier holds a Bachelors of Science Degree in Mechanical Engineering from Purdue University and a Masters of Industrial Engineering in Safety from Texas A&M University.
